Valley Health Partners Community Health Centers is pleased to offer secure access to your health information via a secure, yet easy-to-use patient portal system called MyVHP. These Terms & Conditions outline Valley Health Partners Community Health Centers’s practices and our sensitivity to your right to privacy. Valley Health Partners Community Health Centers reserves the right to revoke access of MyVHP.org at any time for abusive use of the system.

Response to Electronic Messaging - Valley Health Partners Community Health Centers will make its best effort to provide a timely response to electronic inquiries. MyVHP is not to be used for medical emergencies or urgent situations. If you are having a medical emergency, call 911.

Medication refills are processed Monday through Friday during regular business hours. Refills will be completed within two business days. When filled, please call your pharmacy to verify that your prescription is ready for pick-up. Please note that refills can only be processed by the provider who prescribed the medication. Please be aware that online requests for narcotic prescription refills may be denied. Your provider’s office will contact you in the event of a denial.

Messaging to your provider should be used for non-urgent health care needs. Please allow up to 2 business days for a response. Furthermore, with respect to any electronic messages sent by the patient, Valley Health Partners Community Health Centers is only able to respond to such messages based on the information provided by the patient. If there is insufficient information provided, Valley Health Partners Community Health Centers will be unable to provide accurate and reliable services.

Website Links - MyVHP may offer links to related medical websites not managed by Valley Health Partners Community Health Centers. These website link(s) are for a patient’s informational purposes only. Valley Health Partners Community Health Centers does not endorse and has not verified the accuracy of the information in/on these websites, and the patient should not rely on any of the information found on the websites for purposes of treatment or diagnosis.

Minor Eligibility - An individual must be age 18 or older to request a personal account. Parents or guardians of minor children may request access to the minor child’s account by following the terms outlined in the "Proxy Access" section of this Terms and Conditions statement.

Proxy Access - Birth parents or legal guardians may request proxy access to a minor’s (under the age of 18) MyVHP account by completing the Proxy Access Authorization Form at your child’s provider’s office. Proxy access will only be granted to parties with parental rights or legal guardianship over the MyVHP account holder and only to the extent that the party requesting proxy access can demonstrate the legal right to account holder’s medical information. A MyVHP account will be activated for both the proxy and the account holder. If the proxy’s legal relationship with the account holder changes, the proxy must call their healthcare provider’s office. If the proxy does not have an LVHN provider, they should call Valley Health Partners Community Health Centers’s Medical Record Department at 610-994-7500, Monday through Friday from 8:00am to 5:00pm. Valley Health Partners Community Health Centers reserves the right to revoke proxy access at any time for any reason.

Messaging Privacy - Patients who are users of MyVHP should be aware that they will be notified via e-mail or via text message when new medical information is available on MyVHP. Any person with access to a patient’s e-mail will be able to see this notification. This could include the patient’s spouse, employer or anyone else that can access a patient’s e-mail account or cellular phone. Although no private medical information will be sent, the notification that new medical information is available by accessing MyVHP may be information that a patient would not want others to know. Thus, patients should take this into account when providing an e-mail address. Please know that if you send us a message, it may be shared with the Valley Health Partners Community Health Centers clinical staff that assists the physician in providing the patient’s medical care. A patient’s confidential medical information on MyVHP will be accessible only to authorized Valley Health Partners Community Health Centers clinical staff.

Security and Confidentiality - Valley Health Partners Community Health Centers affords the same degree of confidentiality to medical information stored on MyVHP as is given to medical information stored by Valley Health Partners Community Health Centers in any other medium. Valley Health Partners Community Health Centers is committed to protecting the confidentiality of your medical information. Only authorized Valley Health Partners Community Health Centers staff will have access to your medical information. Firewalls, passwords, encryption, and audit trails are further used to safeguard your information. Valley Health Partners Community Health Centers shall identify the records released and note the time and date of access each time the patient accesses MyVHP. Valley Health Partners Community Health Centers has taken steps to make all information received from our online visitors as secure as possible against unauthorized access and use. As a MyVHP user, you agree to maintain the security of your medical information by: 1) Changing your password on a regular basis, and 2) Keeping your login ID and password confidential.

Discontinuing Use of MyVHP - You can stop using MyVHP at any time by contacting your provider’s office by secure message through MyVHP or by phone.

3rd party applications - If you, as a patient of Valley Health Partners Community Health Centers (LVHN), find a 3rd party application that you would like to use to access and import your health information, LVHN provides the ability for you to use this application. To do this, you must establish a MyVHP patient portal account since the security – user name and password – you set for MyVHP would be your authentication for any 3rd party application to access and import your health information.
